As part of the 2025 Home Stretch Symposium in Melbourne, the CREATE Foundation and Home Stretch are proud to celebrate the influence and impact that young people with lived experience are having on out-of-home care system reform. 

This award is to acknowledge innovation and leadership by young individuals, or groups of young people, who have influenced improvements to supports for young people transitioning from care to independence in Australia.

About the CREATE Home Stretch Youth Advocacy Award 

Who can be nominated? (Eligibility) 

Nominations for this award are open to young people, or groups of young people that: 

  • Have lived experience of out-of-home care in Australia. 
  • Are aged 15-25 years old. 
  • Have been working on, advocating for, or leading improvements to transition supports for young people. 
  • Can provide the name and key contact information of the nominee
What’s the selection process?  

All nominations will be reviewed and considered by a selection panel of CREATE Young Consultants and CREATE staff.  

The selection panel will determine the award recipient for 2025 and this will be announced at the Home Stretch Symposium in Melbourne on Thursday the 15 May. 

The selection panel will determine the award recipient based on the following criteria: 

  • Impact and influence on improving transition supports for young people transitioning from care to independence.
  • Creativity and innovation in how they have approached advocacy.
Who can nominate? 

You can self-nominate or you can nominate someone else.  

Are you a young person or group of young people making change?  Nominate yourself. You deserve it! 

Do you know a young person or group of young people making positive change for young people transitioning the indepencence?  Nominate them, they deserve it!
